If the teams could look past trading in-division, San Diego would be a great landing spot for Conforto. Solid lefty OF who should put up at least a 105-110 OPS+ now that he's another year removed from injury. Padres might not be interested in trading Jacob because he throws funny and has put up some promising numbers, but this trade works with any other decent arm on the 40-man or a fringe prospect.
